CERTIFICATE PURCHASE:  A membership certificate is required for membership and is available at a one time cost of $1000.  Payment of certificate purchase is due after being contacted that a membership certificate is available.

ANNUAL DUES:  Seasonal dues are determined by the board of directors annually and are published in the first newsletter of the year. Current is $400 per year.

ALL MEMBERSHIPS INCLUDE THE USE OF THE SWIMMING AND TENNIS FACILITIES OF THE TAYLOR MILL SWIM AND TENNIS CLUB, INC.

HAVING READ THE ABOVE AND AGREEING TO ALL CONDITIONS AS OUTLINED ABOVE, THIS APPLICAION CARRIES BY TRUE SIGNATURE.
